It is amazing how often this happens.
Too much information buried under too many words!
My favorite principal is to keep it short, sharp and simple.
When I get reports from hospitals and other doctors running to more than a page I cringe......
That way chaos and confusion lies.
I have noticed that records from Massachusetts General Hospital are much shorter than the ones from my rural Maine hospitals
I've missed a suspicious breast lesion buried in huge word salad of 2 pages, not in conclusion. Thankfully non malignant.
My referrals are 3 or 4 lines of main points, and past history and medication.
I like the replies to be similar.
A bit like the common Lawyer tactic of burying the opposition in boxes of information hiding the page they need!
But in Medicine's case it is not deliberate, just incompetent!
